Video Squeeze Page Creator

Not sure if this has been shared before or if the owner is a member here, but here's a quick video squeeze creator that's hosted online and renders the finished page in a zip file. Great for lead gathering, video squeezes. When you get to the page click the "Launch the Wizard" button to start creating the VSP.
